本校「學生請假辦法」於111年12月16日「111學年度第1學期學生事務會議」審議通過,自本(111)學年度第2學期起施行,請查照。
說明:
一、辦法修正重點提要:
(一)為協助學生減緩學習壓力及調整學習狀態,新增「心理調適假」(請查閱本辧法第三、五、七條),明定:
1、學生請假單逕送學生事務處生活輔導組備查。
2、請假日數每學期以五天為限。
3、請假日數累計二天者通知導師優先關懷,必要時由導師轉介諮輔相關單位。
4、請假日數連續三天(含)以上者,須檢具醫療院所或相關輔導機構等證明。
5、學生修習課程之學期考試期間,仍依本辦法第六條規定辧理。
(二)配合本校學則第十條規定「授課教師得將學生出勤狀況列為成績評定標準……。」,爰修正學生因行政單位派遣公務申請公假經核准後,應向授課老師報備。
(三)配合教育部「學生懷孕受教權維護及輔導協助要點」第二點、(三)之規定,陪產假之陪產者除配偶外,新增「伴侶」規定。
Important information:
I. Highlights on the latest amendment to the Regulations:
(I) To help students relieve learning pressure and adjust their learning status, the Regulations include the application for mental health leave, with related rules as follows (according to Articles 3, 5 and 7):
1. Students submit a completed application form directly to the Student Assistant Division for administrative verification.
2. Students are allowed to apply for mental health leave for a maximum of five days in a semester.
3. If mental health leave amounts to two consecutive days, the advisor should be notified to provide priority care, and if necessary, the advisor will refer the applicant to the relevant
counseling unit.
4. Applications for mental health leave for three consecutive days or more shall be submitted with a medical certificate issued by the medical institute or related counseling institution.
5. Applications for mental health leave during the end-of-term examination period shall be administered in accordance with Article 6.
(II) Students taking an official leave to perform public duties assigned by an administrative unit shall inform course instructors of the leave upon approval of the application in accordance with Article 10 of NCKU Academic Regulations, stipulating that course instructors may include class attendance in the grading criteria.
(III) Applicants for paternity leave include spouse and partner according to Subparagraph (III) of Direction II in the Guidelines for Protection of the Right to Education and Support for Pregnant Students.
二、注意事項:
(一)學生於請假系統申請後,皆須向師長報備並提醒上線審核,避免假單審核流程超過20天而遭系統退件。
(二)本校教職員工可以成功入口帳號登錄請假系統查看「相關說明文件」,或至生活輔導組網頁查閱學生請假相關規定。
(三)另本校兼任老師登入系統之帳號為「身分證」,密碼為「身分證後四碼 生日後四碼」。
II. Notes:
(I) Upon submission of an application for leave, applicants should remind their instructors and the offices concerned of the approval procedures. An application that fails to be approved within the following 20 days will be automatically rejected online.
(II) NCKU faculty and staff members are allowed to log into the Student Leave Management System with their NCKU Cheng-Kung Portal user accounts to read the submission report of the system and related documents. Meanwhile, they may find the regulations for student leave applications at the website of the Student Assistance Division.
(III) NCKU adjunct faculty members are allowed to log into the system with their national ID number as the user account and a combination of the last four digits of their national ID number and the last four digits of the birthdate (mm/dd) as the password.
